ICS Population Health Place Development Programme Supporting Population-based transformation of Place-based partnerships
A new national programme has been launched which will help to accelerate development of thriving place-based approaches to improving population health.
The Integrated Care System (ICS) Population Health Place Development Programme is funded by NHS England and Improvement and NHSX. The ICS Population Health and Place Development Programme is a voluntary programme – locally-led, nationally-supported –which provides practical support to accelerate and embed adoption of PHM across Integrated Care Systems (ICSs) building on existing efforts and tailored to local needs.
