This purports to be 'a record of agreed best-interests decisions'. It is much better than similar documents from 10, or even 5, years ago. For example it includes:

'We are not aware of other family or interested parties who hold different views to those expressed here'

something very similar in my 'Reason 2'.

However - the form is legally flawed because welfare attorneys are in essence appointed to make best-interests decisions [not to 'contribute to the decisions']. Which is 'not what this form says'.
